It is critical that you enter your legal name, email and cell phone number (if available) so that we may contact you if there is a conflict when requesting your appointment. Please provide information about your VISION PLAN which is almost always different than your medical insurance. All Plans will be verified before we approve your appointment request. Please keep in mind that all appointment requests must be approved by a member of our staff before they are added to the schedule. If we need more information we will send you an email or text message. If you do not respond within 2 hours we will cancel your request.

IF YOU REQUIRE FACILITATION OF ANY KIND (EG: NON ENGLISH SPEAKING, WHEELCHAIR USER, HEARING DIFFICULTY) THIS MAY REQUIRE ADDITIONAL TIME IN OUR SCHEDULE, WE REQUEST YOU DO NOT USE THE ONLINE SCHEDULER FOR YOUR APPOINTMENT. PLEASE CALL 610-913-0126. WE RESERVE THE RIGHT TO RESCHEDULE YOU IF WE FEEL IT MAY CAUSE A BACKUP TO OUR SCHEDULE.
IF YOUR ARE IN NEED OF AN EMERGENCY VISIT CALL OUR OFFICE.

THE SCHEDULER IS FOR VISITS WITH THE DOCTOR ONLY.
